The following video shows Professor Cheng Man-Ching performing the 37 movement form. I'm including these videos because the form we are studying was refined by the Professor so this is good source material. The video quality is highly degraded from who knows how many numbers of video/film transfers. At any rate I think the spirit of the form still shines through.

Here is the form demonstrated by William CC Chen, student of Cheng Man-Ching. As you can see from the description, this is not exactly the same as the Professor's 37 movement form, but for the first quarter or third of the form the movements are identical to what we practice in class. This video might be a little easier as a practice guide as following Mr. Chen from the back should be much like following along in class.

This is a famous film of Cheng Man-Ching doing the 37 movement form. This particular video has been around for many years, usually chopped into pieces and of varying (i.e. poor) quality. This video shows the form in its entirety and the film transfer is of decent quality. Thank you to John M. for locating this video.
